Milling with FILL inside and outside letters or islands

I used to know how to do this, but now I can't remember how to do it. Imagine a plaque with the letter "B". You want to make it stand out my doing a spiral fill outside the letter and again inside the 2 inside segments of the letter. I've assigned a layer name for each part.
layer name: INSIDE_letter
layer name: OUTSIDE_letter
layer name: INSIDE_plaque
And then Process > New fill > appropriate layer
Anytime I choose New fill, it fills everything including the part I want to stand out by not milling.
I am using a very old (but registered) version, but I'm not convinced that it's incapable of doing this operation.
