V-Carve Issue (lead-in I think)

I am trying to carve out the shape shown in picture below using Amana RC-1102 90deg x 3/4" flute x 1/2" shank V-Groove bit (http://bit.ly/2opMTdr) but am running into what looks like a lead in issue. Please see attached pictures of the results and my settings and please advise what is it that I'm doing wrong? I have my max depth set to 1/4".
